Less expensive plans have fewer benefits and higher out-of-pocket costs. More expensive plans include extra benefits, like some Medicare deductibles, additional hospital benefits, at-home recovery, and more. You have to decide what sort of plan makes the most sense for you. If you drop your Medigap policy, there is no guarantee you will be able to get it back.

If you have special health care or financial needs, you may qualify for a Special Needs Plan. All Special Needs Plans include drug coverage. Other benefits may include coordination of care, transportation to and from medical appointments, credits to buy everyday health items, and routine vision and dental coverage. No. Plan G covers less than Medicare supplemental Plan F. You pay your own Part B deductible. However, you get lower premiums for Plan G, and sometimes that makes it a better value. Be sure to compare the numbers. In my opinion, the best Medicare plan is the one that will cost you the least annual out-of-pocket spending and has the lowest rate increases in recent years.


The Monthly Premium for Part B for 2019 is $135.50 per month but anyone on Social Security in 2019 is "held harmless" from that amount if the increase in their SS monthly benefit does not cover the increase in their Part B premium from 2018 to 2019. This hold harmless provision is significant in years when SS does not increase but that is not the case for 2019. There are additional income-weighted surtaxes for those with incomes more than $85,000 per annum.[45]

Massachusetts, Minnesota, and Wisconsin standardize their Medicare Supplement insurance plans differently from the rest of the country. In all states, insurance companies that sell Medicare Supplement insurance aren’t required to offer all plan types. However, any insurance company that sells Medigap insurance is required by law to offer Medigap Plan A. If an insurance company wants to offer other Medigap plans, it must sell either Plan C or Plan F in addition to any other plans it would like to sell.

A Medicare Part D Prescription Drug Plan (PDP) can help pay your prescription drug costs. Designed to work alongside Original Medicare coverage, Medicare Prescription Drug Plans are available from private insurance companies approved by Medicare and doing business in Minnesota. You can also enroll in a Medicare Prescription Drug Plan if you enroll in a Medicare Advantage plan that does not include Part D prescription drug coverage in its benefits.
‡Advantage Plus is available for all Senior Advantage (HMO) and Medicare Advantage (HMO) individual plan members. Advantage Plus is available for all Medicare Plus (Cost) individual plan members, except for Basic plans. In California, Advantage Plus is not available under the Medicare Medi-Cal (HMO SNP) plan. Generally, if you already receive Social Security payments, at age 65 you are automatically enrolled in Medicare Part A (Hospital Insurance) and Medicare Part B (Medical Insurance). If you choose not to accept Part B (typically because you are still working and receiving employer insurance), you must proactively opt out of it when you receive your automatic enrollment package. You may delay Part B enrollment with no penalty under some circumstances (e.g. the employment situation noted above), or with penalty under other circumstances. If you do not receive SS when you turn 65 you must proactively join Medicare if you want it (and the penalties may apply if you choose not to based on various factors).
**NY: In New York, the Excess Charge is limited to 5%; PA and OH: Under Pennsylvania and Ohio law, a physician may not charge or collect fees from Medicare patients which exceed the Medicare-approved Part B charge. Plans F and G pay benefits for excess charges when services are rendered in a jurisdiction not having a balance billing law; TX: In Texas, the amount cannot exceed 15% over the Medicare approved amount or any other charge limitation established by the Medicare program or state law. Note that the limiting charge applies only to certain services and does not apply to some supplies and durable medical equipment; VT: Vermont law generally prohibits a physician from charging more than the Medicare approved amount. However, there are exceptions and this prohibition may not apply if you receive services out of state.

Different insurers cover prescriptions differently, so you may find that one company or another does a better job of helping you pay for your medicine. This might not always be the insurer that offers you the lowest rates for your medical coverage. Note that insurers may change their drug plans each year, so it’s a good idea to make sure that these changes won’t negatively impact you. With Medicare Part C plans in Minnesota, you will have to change all of your coverage if you want to change your drug plan, and with supplement plans, you can just change your drug plan.
In states with lots of rural areas, like Minnesota, Medicare Cost plans tend to be more popular because they offer more flexibility than an HMO. If a plan member gets services inside of the network of Medicare Cost Plans, they work the same way that an HMO works. If the plan member decides to visit a non-network medical provider, Medicare Cost Plans will cover those services the same way that Original Medicare Part A and Part B do. Typically, a Medicare Advantage HMO won’t cover non-emergency services outside of the network at all.

Special Needs Plans (SNP): Special Needs Plans are for beneficiaries with certain unique situations and meet certain eligibility criteria. These plans may limit membership to people who have certain chronic conditions, live in an institution (such as a nursing home), or are dual eligibles (receive both Medicare and Medicaid benefits). You must meet the eligibility requirements of the Special Needs Plan to enroll; for example, to enroll in a Dual-Eligible Special Needs Plan in your service area, you must have both Medicare and Medicaid coverage.
Depending on the state that you live in, you may not be able to get Medicare Supplement coverage if you’re under 65 and have Medicare because of disability, end-stage renal disease, or amyotrophic lateral sclerosis. States aren’t required to offer Medigap coverage to beneficiaries under 65. If you’re under 65 and enrolled in Original Medicare, check with your state’s insurance department to find out if you’re eligible to enroll in a Medicare Supplement plan.
Medigap plans work differently than Advantage plans. Mostly, when you have an Advantage plan, you will use that instead of Part A and Part B. In contrast, Medicare supplement plans work with your basic benefits to fill in some gaps for coinsurance. The advantages of supplements are that they don’t rely upon plan networks and offer stable benefits from year to year. You can use your supplement all over the country, and in some cases, for urgent medical services outside of the country.
Over the long-term, Medicare faces significant financial challenges because of rising overall health care costs, increasing enrollment as the population ages, and a decreasing ratio of workers to enrollees. Total Medicare spending is projected to increase from $523 billion in 2010 to around $900 billion by 2020. From 2010 to 2030, Medicare enrollment is projected to increase dramatically, from 47 million to 79 million, and the ratio of workers to enrollees is expected to decrease from 3.7 to 2.4.[82] However, the ratio of workers to retirees has declined steadily for decades, and social insurance systems have remained sustainable due to rising worker productivity. There is some evidence that productivity gains will continue to offset demographic trends in the near future.[83]

Parts A and B/D use separate trust funds to receive and disburse the funds mentioned above. The Medicare Part C program uses these same two trust funds as well in a proportion determined by the CMS reflecting that Part C beneficiaries are fully on Parts A and B of Medicare just as all other beneficiaries, but that their medical needs are paid for per capita through a sponsor (most often an integrated health delivery system or spin out) to providers rather than "fee for service" (FFS) directly to a provider through an insurance company called a Medicare Administrative Contractor.
Beneficiaries may enroll in a Medicare Supplement insurance plan in California during their six-month Medigap Open Enrollment Period, beginning on the first day of the month that they are 65 or older and enrolled in Medicare Part B. During this time, beneficiaries aren’t subject to medical underwriting, which means they cannot be charged higher premiums or denied coverage based solely on medical history or a current medical condition. However, if a beneficiary adds or changes a Medicare Supplement insurance plan at any other time, medical underwriting guidelines will generally apply.
In 2003 Congress passed the Medicare Prescription Drug, Improvement, and Modernization Act, which President George W. Bush signed into law on December 8, 2003. Part of this legislation included filling gaps in prescription-drug coverage left by the Medicare Secondary Payer Act that was enacted in 1980. The 2003 bill strengthened the Workers' Compensation Medicare Set-Aside Program (WCMSA) that is monitored and administered by CMS.
Price transparency: To get a quote you can either call a United Medicare Advisors representative or complete an online form with your contact and health details. Upon sending it off, an agent will contact you with suggested plans. United Medicare Advisors need personal information to form a tailored quote for each individual. Their website says they save consumers an average of around $634 per year by switching to a new Medigap plan.

As of 2016, 11 policies are currently sold—though few are available in all states, and some are not available at all in Massachusetts, Minnesota and Wisconsin. These plans are standardized with a base and a series of riders. These are Plan A, Plan B, Plan C, Plan D, Plan F, High Deductible Plan F, Plan G, Plan K, Plan L, Plan M, and Plan N. Cost is usually the only difference between Medigap policies with the same letter sold by different insurance companies in the same state. Unlike public Part C Medicare health Plans, Medigap plans have no networks, and any provider who accepts Original Medicare must also accept Medigap.
The intention of both the 1997 and 2003 law was that the differences between fee for service and capitated fee beneficiaries would reach parity over time and that has mostly been achieved, given that it can never literally be achieved without a major reform of Medicare because the Part C capitated fee in one year is based on the fee for service spending the previous year.

A federal law passed in 2003 created a “competition” requirement for Medicare Cost plans, which stipulated the plans could not be offered in service areas where there was significant competition from Medicare Advantage plans. Congress delayed implementation of the requirement several times until a law passed in 2015 that called for the rule to take effect in 2019.
