CMS projections in 2018 estimated that the average basic premium for a Medicare Part D prescription drug plan will fall to $32.50 per month this year from its $33.59 last year. But you need to look beyond the premiums to determine your total costs: Make a list of your prescription medications, then check out your plan’s formularies to make sure your drugs are covered and to learn which tier your drugs are in. (The higher the tier, the higher your copay.) And look at the costs of deductibles and coinsurance, especially if you’re taking expensive specialty drugs.

Each Medicare Supplement insurance plan offers a different level of basic benefits, but each lettered plan must include the same standardized basic benefits regardless of insurance company and location. For example, Medicare Supplement Plan G in Florida includes the same basic benefits as Plan G in North Dakota. Medicare has four basic parts: A, B, C, and D. Taken together, Parts A (hospital care), B (doctors, medical procedures, equipment), and D (prescription drugs) provide basic coverage for Americans 65 and older. What's relevant for this article is what these parts don't cover, such as deductibles, co-pays, and other medical expenses that could wipe out your savings should you become seriously ill. That's where Part C comes in. Also known as Medicare Advantage, it's one of two ways to protect against the potentially high cost of an accident or illness. The other option is Medicare Supplement Insurance, also called Medigap coverage. Here's a look at the two options.
The 10 different Medicare Supplement (Medigap) plans available in most states have standardized benefits across each plan letter. For example, Medigap Plan A will have the same benefits regardless of which state you live in or which insurance company you buy from. If you live in Massachusetts, Minnesota, or Wisconsin, the Medigap plans in these states are standardized differently.
As a Medicare recipient, you have choices for your healthcare. If you still have job-related health insurance, for instance, you may not need to enhance your Medicare benefits and can delay enrollment in either Medicare Advantage Plans or Minnesota Medigap plans. If not, you should know that Part A and Part B of Medicare include many deductibles and copays. 

Part A covers inpatient hospital stays where the beneficiary has been formally admitted to the hospital, including semi-private room, food, and tests. As of January 1, 2018, Medicare Part A had an inpatient hospital deductible of $1340, coinsurance per day as $335 after 61 days confinement within one "spell of illness", coinsurance for "lifetime reserve days" (essentially, days 91-150 of one or more stay of more than 60 days) of $670 per day. The structure of coinsurance in a Skilled Nursing Facility (following a medically necessary hospital confinement of 3 nights in row or more) is different: zero for days 1-20; $167.50 per day for days 21-100. Many medical services provided under Part A (e.g., some surgery in an acute care hospital, some physical therapy in a skilled nursing facility) is covered under Part B. These coverage amounts increase or decrease yearly on 1st day of the year.
The Centers for Medicare and Medicaid Services (CMS), a component of the U.S. Department of Health and Human Services (HHS), administers Medicare, Medicaid, the Children's Health Insurance Program (CHIP), the Clinical Laboratory Improvement Amendments (CLIA), and parts of the Affordable Care Act (ACA) ("Obamacare").[12] Along with the Departments of Labor and Treasury, the CMS also implements the insurance reform provisions of the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and most aspects of the Patient Protection and Affordable Care Act of 2010 as amended. The Social Security Administration (SSA) is responsible for determining Medicare eligibility, eligibility for and payment of Extra Help/Low Income Subsidy payments related to Parts C and D of Medicare, and collecting most premium payments for the Medicare program.

Congress also attempted to reduce payments to public Part C Medicare health plans by aligning the rules that establish Part C plans' capitated fees more closely with the FFS paid for comparable care to "similar beneficiaries" under Parts A and B of Medicare. Primarily these reductions involved much discretion on the part of CMS and examples of what CMS did included effectively ending a Part C program Congress had previously initiated to increase the use of Part C in rural areas (the so-called Part C PFFS plan) and reducing over time a program that encouraged employers and unions to create their own Part C plans not available to the general Medicare beneficiary base (so-called Part C EGWP plans) by providing higher reimbursement. These two types of Part C plans had been identified by MedPAC as the programs that most negatively affected parity between the cost of Medicare beneficiaries on Parts A/B/C and the costs of beneficiaries not on Parts A/B/C. These efforts to reach parity have been more than successful. As of 2015, all beneficiaries on A/B/C cost 4% less per person than all beneficiaries not on A/B/C. But whether that is because the cost of the former decreased or the cost of the latter increased is not known.


Basic Plan helps cover Medicare's Parts A and B coinsurance, hospice care coinsurance or copayment, skilled nursing facility care coinsurance, the first 3 pints of blood each year, and Wisconsin Mandated Benefits when not covered by Medicare. Basic Plan with Copay covers the same benefits as Basic Plan for Medicare Part A. For Medicare Part B medical expenses, the plan pays generally 20%, other than up to $20 per office visit and up to $50 per emergency room visit. The copayment of up to $50 is waived if you are admitted to any hospital and the emergency visit is covered as a Medicare Part A expense. This plan also covers the Wisconsin Mandated Benefits when not covered by Medicare.
Massachusetts, Minnesota, and Wisconsin standardize their Medicare Supplement insurance plans differently from the rest of the country. In all states, insurance companies that sell Medicare Supplement insurance aren’t required to offer all plan types. However, any insurance company that sells Medigap insurance is required by law to offer Medigap Plan A. If an insurance company wants to offer other Medigap plans, it must sell either Plan C or Plan F in addition to any other plans it would like to sell.
Core Plan covers the Part A coinsurance plus coverage for the first 90 days per benefit period (not including the Medicare Part A deductible) and the 60 Medicare lifetime reserve days, plus coverage for 365 additional days after Medicare benefits end, Part B coinsurance, first 3 pints of Blood each year, Part A Hospice coinsurance and certain state-mandated services.
Although private insurance companies are required to offer the same basic benefits for each lettered plan, they do have the ability to charge different premiums. Another wrinkle is that people who want a supplement might have a better chance of getting into the coverage during the transition out of their Medicare Cost plan, when the supplement is provided on a “guaranteed issue” basis. Later, insurance companies can ask questions about a senior’s health status and deny coverage depending on the answers, said Greiner of the Minnesota Board on Aging.
The highest penalties on hospitals are charged after knee or hip replacements, $265,000 per excess readmission.[34] The goals are to encourage better post-hospital care and more referrals to hospice and end-of-life care in lieu of treatment,[35][36] while the effect is also to reduce coverage in hospitals that treat poor and frail patients.[37][38] The total penalties for above-average readmissions in 2013 are $280 million,[39] for 7,000 excess readmissions, or $40,000 for each readmission above the US average rate.[40]
Senior supplements are supplemental health insurance plans designed specifically for senior’s needs. Supplemental health insurance includes products like dental, vision, and life insurance. Since the mid-1990s, there have been a number of proposals to change Medicare from a publicly run social insurance program with a defined benefit, for which there is no limit to the government's expenses, into a publicly run health plan program that offers "premium support" for enrollees.[119][120] The basic concept behind the proposals is that the government would make a defined contribution, that is a premium support, to the health plan of a Medicare enrollee's choice. Sponsors would compete to provide Medicare benefits and this competition would set the level of fixed contribution. Additionally, enrollees would be able to purchase greater coverage by paying more in addition to the fixed government contribution. Conversely, enrollees could choose lower cost coverage and keep the difference between their coverage costs and the fixed government contribution.[121][122] The goal of premium Medicare plans is for greater cost-effectiveness; if such a proposal worked as planned, the financial incentive would be greatest for Medicare plans that offer the best care at the lowest cost.[119][122]
If you forgo supplemental insurance, then you’re responsible for out-of-pocket health insurance costs – that can quickly cost thousands of dollars. If you’re on a fixed income, this may quickly eat into your retirement savings. In states with lots of rural areas, like Minnesota, Medicare Cost plans tend to be more popular because they offer more flexibility than an HMO. If a plan member gets services inside of the network of Medicare Cost Plans, they work the same way that an HMO works. If the plan member decides to visit a non-network medical provider, Medicare Cost Plans will cover those services the same way that Original Medicare Part A and Part B do. Typically, a Medicare Advantage HMO won’t cover non-emergency services outside of the network at all.
Payment for physician services under Medicare has evolved since the program was created in 1965. Initially, Medicare compensated physicians based on the physician's charges, and allowed physicians to bill Medicare beneficiaries the amount in excess of Medicare's reimbursement. In 1975, annual increases in physician fees were limited by the Medicare Economic Index (MEI). The MEI was designed to measure changes in costs of physician's time and operating expenses, adjusted for changes in physician productivity. From 1984 to 1991, the yearly change in fees was determined by legislation. This was done because physician fees were rising faster than projected.
